The Resident Evil 4 remake challenges players with various environmental brain teasers that are essential for story progression. During Chapter 4, players must explore the expansive Lake region to locate two hidden stone heads required to unlock the Church Key. These key items are locked behind intricate symbol mechanisms found inside the Large Cave Shrine and the Small Cave Shrine.

Solving the Large Cave Shrine Mechanism

The Large Cave Shrine is situated within a network of tunnels south of the Lake area. Upon reaching the locked door, you will encounter a circular pedestal containing eight distinct symbol buttons. To unlock the gate, players must press three specific buttons that correspond to the yellow environmental paintings scattered around the cavern.

Finding these symbols requires paying close attention to your perspective and the surrounding architecture. The first symbol depicts two curved lines facing each other and is split across two separate stone columns. The remaining two symbols include a tower-like graphic located near the ladder and a distinct circular emblem painted on the lower cave walls. Pressing these three corresponding buttons on the pedestal opens the door, allowing you to collect the Apostate Head.

Cracking the Small Cave Shrine Puzzle

Navigating to the northeast corner of the Lake brings players to the Small Cave Shrine, which features a similar button-operated locking device. Although this area contains fewer enemies, finding the correct markings requires standing at very precise viewing angles. The yellow graffiti icons are deliberately distorted by the uneven wooden structures and rock formations.

The solution requires identifying three specific patterns painted across the environment. You need to locate a symbol resembling three squiggly lines near the base of the ladder, followed by a bird-like shape visible from the upper floor. The final icon looks like three connected chains and can be seen by looking through the shack window. Inputting these three symbols into the device grants access to the room holding the Blasphemer Head.

Unlocking the Mural Cave Pedestal

Once both stone heads are safely in your inventory, you must sail your boat back to the central Mural Cave. This location houses the main altar that holds the primary insignia needed to rescue Ashley from her imprisonment. The pedestal features two giant stone hands designed to hold the items you just recovered.

Approaching the altar allows you to place both the Blasphemer Head and the Apostate Head into the open palms. The order in which you place the stone heads does not impact the final outcome of the puzzle. Once both items are slotted into position, the central mechanism will click open and award you the crucial Church Insignia.
